There’s a kind of autoimmune disease that safety protocols get, where they start creating risk on their own. A false evacuation signal is a lot like white blood cells acting up for no reason.
Curious if other people have examples of protocols backfiring or eating their own tail. With safety/security/health protocols, autoprotocol disease easy to spot, but I imagine it happens with governance, networks, comms too
